Visa Application Checklist 

U.S. Embassy in Bangladesh processes all kind of visa as Immigrant & Non Immigrant (Business/Tourist Visa, Work Visa, Student Visa, Exchange Visitor Visa, Transit/Ship Crew Visa, Religious Worker Visa, Domestic Employee Visa, Journalist and Media Visa) of USA in Bangladesh. To submit visa application, Applicants have to attend in person for interview at The Embassy & submission procedure is appointment basis. GOAL tours & travels provide consultancy & other services for short term visa applications only.

Requirements:

1. Current (valid for at least 6 months & at least 2 blank page) and Old Passports
2. Two recent color photograph with white background (2X2 inch)
3. Visa application confirmation letter
4. Appointment confirmation letter
5. Bank money receipt original copy

Required Information to Fill up Visa Application:

1. What is the purpose of your trip?
2. Full Name (As in Passport) & Passport Number?
3. Have you ever used other names (i.e. maiden, professional religious etc.)
4. Marital Status: (Married/Single/widowed/Divorced/Separated)
5. National identification Number (lf Applicable)
6. Do you hold or have you held any nationality other than the one indicated? (Give the Nationality &Passport No.)
7. U.S. Social Security Number. (lf Applicable)
8. U.S Tax payer lD Number.(lf Applicable)
9. Home Address (Present) & Mailing Address. (Home/Mobile/Business)
10. Telephone Number (Personal & work)
11. E-Mail Address. (lf Applicable)
12. Have you ever lost a passport or had stolen.(Explain with Passport number)
13. When do you intend to arrive in the U.S.?
14. How long do you intend to stay in the U.S.?
15. At what address will you stay in the U.S.?
16. Who will pay for your trip? Give the Name, Relation, and Address & Phone Number.
17. Names and relationships of persons traveling with you.(lf Applicable)
18. Have you ever been in the U.S.? When (Date of Arrival), for how long? (Last five U.S. visit)
19. U.S. Driver’s license Number and State of Driver’s license. (if Applicable)
20. Have you ever been issued a U.S. visa? (lf YES give Date last visa was issued visa class and Visa Number (8 digits)
21. Have you ever been Lost/refused/cancelled/revoked a U.S. visa? When? Where? What type of visa? (if Applicable)
22. Has anyone ever filed an immigrant petition on your behalf with the United States Citizenship and Immigration Services? Explain
23. Full Name and Address and Relationship of Contact Person or Organization in the U.S. (include Zip code, Telephone Number and E-mail)
24. Father full name and date of birth.
25. Mother's full name and date of Birth.
26. Are any of the following persons in the U.S.? (Father/Mother/Son/Daughter/Husband/wife/Brother/Sister? lf YES gives the Name, Relation and status. (i.e. LPR, U.S. citizen, studying, working etc.)
27. Do you have any other relatives in the United States? Yes/No
28. Spouse Full Name, Place& Date of Birth,(lf Applicable)
29. Names, Address and phone number of Present Employer or School.
30. Monthly Salary in local currency & Designation,
31. Provide your employment information for the last FIVE years, if Applicable. Give last Employer Name, Address, Telephone number, Job Title, Supervisor's Name, dates of Employment (dd/mm/yyyy)
32. List all educational institutions you attend or have attended (lf Applicable). Give the Name of Institutions, Address, Course of Study and Dates of Attendance (dd/mm/yyyy).
33. Provide a List of Languages You Speak (Language name)
34. List all countries you have entered in the last FIVE Years (Names only)
35. List all Professional, Social and Charitable Organizations to which you belong.
36. Do you have any specialized skills or Training, including firearms? If YES please explains.
37. Have you ever performed military service? Give the Branch, Rank, Specialty and dates of service.
38. List two contacts in Applicant's Country of residence who can verify information about applicant (Do not list immediate family members or other relatives). Give Address and Phone Number.
39. Do you intend to study in the U.S.? lf YES give the course of study, name and complete address of the School.
40. Do you intend to work in the U.S.? lf YES give the name, complete address of the U.S. employer and Monthly salary (in USD).
41. Name of person/company who filed petition. Give application/petition number
42. Crew Information (Vessel/Aircraft) o Job title, Company name, Company Telephone number, Agency name, contact person, address, phone number, vessel name and identification number.

43. All contact numbers used last 5 years
44. All Email ID used last 5 years
45. Social media account ID as FACEBOOK, TWITER etc.

Important:

1. 1^st time applicants have to attend in interview in Embassy & pick up passport from Embassy’s authorized VFS center.
2. 2^nd time applicants (who have previously used visa & visa expiration time not exceed 48 months) have to Submit passport & delivery in VFS center by “DROP BOX” procedure.
